Job Summary
To provide critical psycho-social support to Palliative Care patients and their families through the Palliative Care consult team. To utilize professional background and skills to assist the team and facilitate patient-family meetings. To assist team, attending physicians, staff, and patient and family in the clarification of the goals of care and the plan of care.Accountabilities
Supervisory/Management Responsibility
This is a non-management job that will report to a supervisor, manager, director, or executive.
Minimum Requirements
Master's Degree in Social Work from an accredited school and three (3) years of experience in a health-related area with a minimum of two(2) of those years working with the frail or elderly population required. Work experience may NOT substitute for education requirement.
Required Certifications/Registrations/Licenses
Licensed Master Social Worker (LMSW) or Licensed Independent Social Worker (LISW) required.
Valid driver's license, an acceptable three-year motor vehicle record as defined by the Acceptable Motor Vehicle Record (MVR) Chart, and a certificate of auto insurance.
In Lieu Of The Above Minimum Requirements
Team members employed in this job prior to July 1, 2020 are grandfathered under prior educational and experience requirements. In addition, team members must be certification eligible and will have one year to obtain required certification.
Other Required Skills and Experience
Individual and family therapy skills; analytical thinking and problem solving abilities.\u00A0 Driving is required for this position using the employee\u2019s personally owned vehicle.\u00A0 Employee must have reliable transportation and will be expected to adhere to the Prisma Health Driver Safety Policy and specific department driving policies.\u00A0 Employee must pass driver training.
