Job Summary

Coordinates communication and medical services between clinical staff, physicians, DMEs, and laboratories. Responsible for coordinating with other disciplines and agencies involved with patient's care.

Accountabilities

  • Team Responsibilities:\u00A0 Promotes,\u00A0 maintains and documents communication between physicians and clinical staff\u00A0 regarding patient care and agency function.\u00A0 Confers with supervisors and attends team meetings. 20%
  • Documentation Responsibilities:\u00A0 Documents orders received from physicians and lab results received from laboratories. 20%
  • Professional Responsibilities:\u00A0 Promotes and expedites communication between home health clinical staff in the field and physicians and their medical staff regarding patient care and agency function.\u00A0 Coordinates patient lab schedules, lab orders, and lab results with clinical staff, laboratories, and physicians.\u00A0 Transmits patient clinical notes to DMEs as required for quality patient care. Maintains the highest level of professionalism and conduct at all times.\u00A0 Follows agency policies and procedures. 20%
  • Clinical Responsibilities:\u00A0 Monitors results of telehealth monitors and reports results as appropriate to clinical staff and the patient's physician. 20%
  • Administrative Responsibilities:\u00A0 Submits all paperwork/documentation according to agency policy.\u00A0 Participates in chart audits and other related Quality Improvement projects. 15%
  • Accepts assignments outside normal duties when such fulfills an agency need. 5%

Minimum Requirements

Associate's Degree

Graduate of an accredited school of Nursing program and approved by the South Carolina State Board of Nursing

Bachelor's degree in Nursing preferred.

One year experience as a registered nurse

Required Certifications/Registrations/Licenses

Licensure to practice as a Registered Nurse in South Carolina

Valid driver\u2019s license, an acceptable three-year motor vehicle record as defined by the Acceptable Motor Vehicle Record (MVR) Chart, and a certificate of auto insurance.

Required Knowledge, Skills, Abilities

Basic computer skills

Knowledge of office equipment (fax/copier)

Proficient computer skills (word processing, database)

Data entry

Other Required Skills/Experience

Driving is required for this position using the employee\u2019s personally owned vehicle.\u00A0 Employee must have reliable transportation and will be expected to adhere to the Prisma Health Driver Safety Policy and specific department driving policies.\u00A0 Employee must pass driver training.
