Job Summary

Provides physical therapy treatment under the direction of a Registered Physical Therapist. Provides patient/family education, completes documentation of patient treatments, charges, and other information as needed. Collaborates with an interdisciplinary team in the implementation of the patient's individualized plan of care

* Implements and manages a specific and comprehensive treatment plan.

\u00A0\u00A0 -Works under the supervision of a registered physical therapist in delivering patient treatment.

\u00A0\u00A0 -Demonstrates advanced skills in administering physical therapy procedures.

\u00A0\u00A0 -Assesses patient response to treatment, modifying when appropriate with input from the supervising therapist.

\u00A0\u00A0 -Demonstrates awareness and use of proper safety measures regarding patient, others in the treatment area, and\u00A0\u00A0\u00A0

\u00A0\u00A0 -Interacts regularly with others involved in the patient's care to incorporate an interdisciplinary treatment approach

\u00A0\u00A0\u00A0 for quality, coordinated care.\u00A0 55%

* Complies with departmental policies for documentation

\u00A0\u00A0 -Documents all aspects of assessment, planning, treatment, and discharge in the medical record (and in the working

* Participates in patient discharge preparations.

\u00A0\u00A0 -Provides Case Managers with appropriate DME referrals in an adequate amount of time in which the Case Manager

\u00A0\u00A0\u00A0 is able to secure the equipment for the patient prior to D/C from BEH.

\u00A0\u00A0 -Provides information regarding follow-up services which may include a referral to other community resources.\u00A0 5%

* Actively participates in patient and family education process.

\u00A0\u00A0 -Provides education and training to patients, families, and caregivers regarding diagnoses, progress, long-term\u00A0\u00A0\u00A0\u00A0\u00A0\u00A0

Minimum Education
